Submit an Authorization Form to the Calvin Finance Office
- Complete and submit a signed Authorization Form to the finance office authorizing Calvin Presbyterian Church to set up one-time or recurring electronic contributions. These can be:
- withdrawals from your checking or savings account (ACH)
- charged to your debit or credit card.
Recurring gifts can be for a specific number of times or until you request they be changed or stopped.
To designate to a specific fund, include this information on the EFT authorization form.
Calvin will pay a fee of about 25 cents for these transactions.
